How do I order a test?

For healthcare providers: If this is your first time ordering from AccessDx, please complete our new account form.

Once we receive your New Account Form, we will send you instructions for logging in to the AccessDx Provider Portal to place orders for kits and tests and view results.

You can also order tests by completing and returning the PGx Profile Test Requisition Form included with your test kitsand we will provide cheek swab collection kits with prepaid return shipping labels.

For patients: Your healthcare provider needs to order testing on your behalf. He or she will need to complete the steps above.

How do I request additional specimen collection kits?

Please contact our customer service team at [email protected] to request more kits. You will need to provide:

  • The number of kits requested
  • The address where the kits should be delivered

What genes are included on the AccessDx PGx Profile panel?

The AccessDx PGx Profile panel analyzes more than 120 variants across 38 genes: ABCB1, ABCG2, ACYP2, ADRA2A, ANKK1, APOE, CEP72, COMT, CYP1A2, CYP2B6, CYP2C, CYP2C19, CYP2C8, CYP2C9, CYP2D6, CYP3A4, CYP3A5, CYP4F2, DPYD, DRD2, F2, F5, FKBP5, G6PD, GRIK4, HCP5/HLA-B*57:01, HTR2A, HTR2C, IFNL3/IFNL4, MTHFR, NUDT15, OPRM1, SLC6A4, SLCO1B1, TPMT, UGT1A1, UGT2B15, and VKORC1.

Three additional HLA tests are available upon request: HLA-A*31:01, HLA-B*15:02, and HLA-B*58:01.

How long does it take to receive PGx test results?

Results are usually available 7-10 days after the sample receipt at the lab.
